CLUB GENIUS SUMMER FUN DAY CAMP PROGRAM

 

 AGREEMENT AND POLICIES—Monday through Friday 8:00 a.m. to 6:00 p.m.

 

Please read carefully and initial each statement. 

 

____Parents are required to escort their child into the club and sign him/her in on the sign in sheet.

Do not drop your child off outside the club.

 

____When parents pick up the child; it is required for the child to be signed out by the parent.   

 

____It is assumed that if the child stays after 2:00 p.m. the child will be participating in the

            conventional Boys and Girls Club program.  Please read the “Open Door Policy” statement

            closely.

 

____   Lunch will be at 12:00 p.m.  You are expected to send a sack lunch

You are encouraged to provide your child with snacks if he/she is staying past 2:00 p.m.

Snack Bar is also available during the afternoon.

 

____   If your child is on medication, parent must provide a written explanation of dosage and

           possible side effects.  Also, please notify the staff of any food allergies that your child might

           have. 

 

____The child must attend the morning program at least 3 times a week in order to attend field trips.

            NO EXCEPTIONS!

 

____Parent received and filled out the registration form.

____If your child is not picked up by 6:00 p.m., there will be a late pick-up fee charge of $5.00 for

every 15 minutes.

 

____The Club Genius Summer is available from June 14, 2010 to August 6, 2010

         (tentative dates – subject to change depending on participation).

 

 

 CLUB GENIUS PROGRAM PRICE LIST

 

 

____   SILVER PACKAGE

$75.00 per week per child – does not include fieldtrips
            (must bring lunch/snacks). Plus $25.00 Enrollment Fee

 

 

*** SNACK BAR OPENED AFTER 2:00 PM ***

Typical Snack Bar costs:

*  Food Items (Mac ‘n Cheese, Burritos, Corn Dogs, Hot Dogs, Fruit, etc) $1.00 - $3.00

*  Fruit Drinks, Gatorade, Flavored Bottled Water .75˘ - $1.00

Typical local Fieldtrips are $5.00 per trip (movies, swimming, fishing, water park, etc).

Out of Town Fieldtrip pricing will be available when scheduled.
